If your student will need to take medication at school, please bring the completed forms with the medication to Information Day, or email Nurse Lacey for an appointment.  All medication, including over-the-counter medication, must be accompanied by a medication administration form including parent and doctor signatures.  Prescription medication must have a pharmacy label that matches the information on the medication form.  Over-the-counter medication must arrive in a new, unopened container.  All medication forms and emergency plans can be found at the Edmond Public Schools website HERE.
